An established outpatient healthcare organization in New York is seeking an experienced NP/PA to join a collaborative and mission-driven clinical team serving diverse patient populations.

Responsibilities:
- Conduct comprehensive patient evaluations, including medical histories, physical examinations, and preventive screenings such as women s health exams and pap smears.
- Develop individualized care plans, order and interpret diagnostic testing, and prescribe medications and treatment plans as clinically appropriate.
- Partner closely with physicians, medical assistants, nurses, and support staff to ensure high-quality coordinated patient care.
- Provide patient education in a clear, compassionate, and culturally sensitive manner to improve long-term health outcomes.
- Maintain timely and accurate documentation within the electronic medical record (EMR) system.
- Support chronic disease management through ongoing patient follow-up and continuity of care.
- Participate in quality improvement initiatives, staff meetings, and collaborative clinical discussions.
- Coordinate referrals to specialists and community resources when needed.
- Utilize standing orders and collaborate with clinical support staff to optimize workflow efficiency and patient experience.
- Foster a positive team culture built on communication, accountability, and continuous learning.
Qualifications:
- Active New York State NP or PA license and registration required.
- Minimum of 2 years of experience practicing as a Nurse Practitioner or Physician Assistant in primary care, family medicine, internal medicine, outpatient, or community health settings.
- Previous Women s Health or OB/GYN experience highly preferred.
- Strong communication and interpersonal skills with a collaborative approach to care.
