Responsibilities:

  • Manage a patient load of approximately 17-20 adult patients per day, delivering quality primary care services.
  • Conduct physical exams, diagnose, and treat acute and chronic conditions.
  • Facilitate patient education and health promotion.
  • Collaborate with physicians and other team members to ensure coordinated patient care.
  • Participate in on-call rotation, including weekends and holidays, via text or phone triage.
  • Document all patient interactions accurately in the electronic health record system.
  • Adhere to practice policies and meet required clinical standards.
  • Maintain and expand clinical knowledge to ensure up-to-date patient care.

Requirements:

  • NCCPA certification and current Connecticut licensure.
  • Minimum of 2 years of experience in adult primary care; new graduates with relevant clinical exposure are encouraged to apply.
  • Strong clinical skills in managing adult patients with diverse health needs.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Willingness to participate in on-call duties, including weekends and holidays.
  • Basic proficiency with common EHR platforms.
