Social Worker, Hospice MSW, LCSW,

AccentCare, a leader in home health care services, is seeking a compassionate and experienced Social Worker, Hospice MSW, LCSW to enhance the quality of life for our hospice patients. This role involves providing expert emotional support, crisis intervention, and guidance to patients and their families dealing with terminal illness and end-of-life circumstances. The ideal candidate will use their specialized knowledge and skills to develop tailored care plans that meet the unique needs of each patient, promoting comfort and dignity in their final stages of life. Assess patient and family psychosocial needs and mental health status. Develop and implement comprehensive care plans in collaboration with a multidisciplinary team. Provide crisis intervention and counseling to patients and families. Facilitate communication between patients, families, and healthcare providers to ensure consistency in care. Document patient progress and interventions in patient records appropriately. Advocate for patient needs to ensure the provision of required services. Lead support group discussions and provide education on coping strategies. Proficiency in individual and group therapy Strong interpersonal and communication skills Ability to handle high-stress, emotionally charged situations Experience in crisis intervention Thorough understanding of the healthcare system and patient rights Medical Insurance Dental Insurance Vision Insurance 401k Licensed Clinical Social Worker (LCSW) required. Certification in hospice and palliative care preferred.
