Social Worker (PACE) - HOT SPRINGS (Hiring Immediately)

Department: PACE Hot Springs Shift: Day Working Hours: 8:00am-5:00pm Summary: Responsible for the direct social work case management services to PACE participants. Provides social service support to include participant screening and assessments, case management, counseling and referral. Other information: ? Master's degree required ? LMSW or LCSW required ? Two years experience in healthcare preferred ? Experience working with the frail and elderly population preferred ? Excellent customer service skills ? Excellent interpersonal communication skills This job will be authorized 80.00 hours bi-weekly.
