Encompass Health is seeking a dedicated Physical Therapist to join our team of healthcare professionals in providing exceptional care within our integrated healthcare services network. This role involves working both in facility-based settings and in patient homes, with a focus on restoring patient mobility, alleviating pain, and preventing or limiting permanent physical disabilities. Our Physical Therapists are crucial in helping patients recover from injuries and manage health conditions, ensuring that they achieve their highest level of independence. Assess patient condition by examining muscle and joint performance and functional capacity. Develop treatment plans by identifying short and long-term goals, and selecting therapeutic interventions. Restore patient functionality by using exercises, stretching maneuvers, hands-on therapy, and equipment. Maximize patient independence by establishing patient care goals; teach patients and families to understand conditions, medications, and self-care skills. Ensure the continual evaluation of patient progress and revitalize therapy plans accordingly. Keep accurate documentation and reports of patient care, condition, responses, and progress.
