CTO TOC LPN Care Manager

We are currently seeking a dedicated and experienced CTO TOC LPN Care Manager to join our team. In this role, you will be crucial in managing transitioning of care (TOC) and coordinating patient outcomes effectively. Our ideal candidate is someone who thrives in a fast-paced environment and is passionate about improving patient health through exceptional care planning, communication, and implementation skills. Develop and implement care plans for patients transitioning out of hospital care Coordinate with multidisciplinary teams to ensure seamless patient care Monitor patient progress and adjust care plans based on patient needs and outcomes Educate patients and their families on post-discharge care and health management Assist in the management of the care team's schedules and responsibilities Ensure compliance with all healthcare regulations and standards Strong interpersonal and communication skills Proficient in patient care coordination Adept in using healthcare management software Ability to handle multiple tasks and prioritize effectively Knowledge of healthcare regulations and standards Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid LPN (Licensed Practical Nurse) licensure Certified Care Manager (CCM) is preferred but not required
