Physical Therapist Home Health

HCA Healthcare, a leading provider of patient-centered medical care in the United States, is seeking a dedicated Physical Therapist for our Home Health division. In this role, you will be responsible for providing direct patient care in home settings, assessing patient needs, and developing treatment plans to improve mobility, relieve pain, and prevent or limit permanent physical disabilities in patients suffering from injuries or disease. Assess patient condition by examining muscle strength, mobility, and other motor functions. Formulate treatment plans to address patient needs, goals, and outcomes. Implement physical therapy interventions and monitor patient progress. Educate patients and families on continued care strategies and preventative measures. Maintain patient records and documentation per company guidelines and legal requirements. Collaborate with other healthcare professionals to ensure comprehensive care. Strong interpersonal and communication skills Ability to perform and interpret physical assessments Skill in developing and implementing effective treatment plans Proficiency with physical therapy equipment and techniques Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed as a Physical Therapist in the state of practice Valid driver’s license and reliable transportation CPR certified Experience with electronic health record (EHR) systems preferred
