This position offers a $10,000 sign-on bonus and involves administering physical therapy in a home setting, where you will assist patients in achieving their highest level of functionality following illness, surgery, or injury. Conduct patient assessments and evaluate their physical condition. Develop individualized treatment plans based on each patient's needs and capabilities. Implement therapeutic interventions to improve mobility and alleviate pain. Monitor patient progress and adjust treatment plans as necessary. Educate patients and family members about treatment plans and ongoing management strategies. Maintain accurate patient documentation for all clinical activities and treatments. Collaborate with other team members to ensure a comprehensive approach to patient care. Proficient in developing and executing personalized PT treatment plans. Strong interpersonal communication skills suitable for home visits. Ability to handle physical demands of the job, including the ability to move or lift patients and equipment.
