Home Health Admissions Registered Nurse RN Full Time

Aveanna Healthcare, a premier national provider of pediatric and adult home care services, is seeking a compassionate and dedicated Home Health Admissions Registered Nurse (RN) to join our team. This position offers the opportunity to work directly with patients in their homes, ensuring they receive the personalized care needed to manage their conditions and improve their quality of life. The ideal candidate will be responsible for initial patient assessments, care planning, and coordination with other team members to facilitate effective home health services. Conduct comprehensive patient assessments during initial home visits to determine healthcare needs and appropriate plans of care. Collaborate with physicians, therapists, and other healthcare professionals to implement effective care plans. Educate patients and their families on disease processes, treatment plans, and health maintenance routines to promote optimal outcomes. Administer medications and treatments as prescribed and monitor for effectiveness and side effects. Document patient care services by charting in patient and department records according to professional standards and regulatory guidelines. Strong clinical skills with an ability to assess and manage patient care effectively Excellent communication and interpersonal skills for dealing with diverse patients and families Ability to work autonomously and manage time effectively Proficiency with electronic medical records (EMR) systems Medical Insurance Dental Insurance Vision Insurance 401k Valid RN license in the state of practice Basic Life Support (BLS) Certification Driver's license and reliable transportation as this role requires traveling to patient homes
