RN- Registered Nurse Patient Navigator

Ascension is seeking a compassionate and dedicated RN - Registered Nurse Patient Navigator to join our faith-based healthcare organization. In this role, you will be pivotal in guiding patients through the complexities of the healthcare system, ensuring they receive timely care and support. Leveraging Ascension's extensive network of hospitals, clinics, and healthcare professionals, you will provide personalized care plans that reflect our commitment to transforming healthcare through innovation and compassionate service. Serve as a primary point of contact for patients, coordinating care and navigating patients through various healthcare services. Assess patients' needs, develop, implement, and evaluate care plans collaboratively with healthcare teams. Educate patients and their families about health conditions, treatment options, and healthcare resources available. Facilitate communication between patients, family members, and various healthcare professionals to ensure clarity and continuity of care. Monitor patient progress and outcomes, adjusting care plans as needed to meet health objectives. Advocate for patients' needs and preferences with healthcare teams and external resources. Maintain comprehensive documentation of patient interactions, treatments, and outcomes in compliance with HIPAA regulations and organizational policies. Strong interpersonal and communication skills Ability to manage multiple cases and priorities Knowledge of healthcare systems, regulations, and policies Proficiency in electronic health record (EHR) systems Critical thinking and problem-solving skills Compassion and empathy towards patients Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of employment. Certification in patient navigation is preferred but not required.
