RN MDS

Good Samaritan Health Center is seeking a dedicated and detail-oriented RN MDS (Minimum Data Set) Coordinator to join our team. This role is crucial in ensuring our health center delivers personalized and efficient care to our underserved communities, aligning with our mission of promoting holistic health outcomes. The RN MDS Coordinator will be responsible for assessing and analyzing the care needs of our residents, utilizing the MDS tool as mandated by federal and state guidelines to formulate individualized care plans that enhance our patients' quality of life. Conduct comprehensive assessments using the Minimum Data Set (MDS) for resident care planning and auditing purposes. Collaborate with interdisciplinary teams to develop and review personalized care plans. Ensure compliance with federal and state regulations regarding documentation and care planning. Train and guide healthcare staff on MDS processes and updates. Monitor and report changes in patient health status and adjust care plans as necessary. Strong knowledge of MDS 3.0 RAI Manual and CMS guidelines. Excellent assessment and documentation skills. Ability to work collaboratively with a multidisciplinary team. Strong organizational and communication skills. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license. MDS Certification preferred.
