Physical Therapist Homecare, Silver Spring

MedStar Health, a leader in patient-first healthcare in the D.C.-Maryland-Virginia region, is seeking a dedicated Physical Therapist for our homecare division in Silver Spring. This role involves providing direct therapy and rehabilitation services to patients in their homes, tailoring interventions to meet individual needs, with an emphasis on functional mobility and pain management. You will collaborate with a multidisciplinary team to create comprehensive, patient-centered treatment plans that allow for optimal patient outcomes. Assess patient health condition and therapy needs via direct consultations Develop tailored treatment plans focusing on achievable physical therapy goals Implement therapy interventions to improve patient mobility and manage pain Educate patients and families on treatment plans and home exercises Regularly reevaluate and adjust treatment plans based on patient progress Document treatment and patient progress comprehensively and timely Coordinate with healthcare providers to ensure a holistic approach to patient care Knowledge of physical therapy principles and practices Ability to handle a variety of patient needs and complex cases Efficient in time management and the ability to manage multiple patient cases Strong interpersonal and communication skills Adept in using physical therapy tools and equipment Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid state Physical Therapy License CPR Certification preferred
