Job Description Out Patient Family Medicine Seeking a Family Practice physician to work with a well established, nationally recognized community health facility located in Chicago. Office is comprised of family medicine, pediatrics, OB/Gyn, behavior health, nursing staff and a pharmacy. They are seeking a physician that shares their mission and passion for working in a community health environment. They will offer a flexible schedule: 5 -8hr days with allotted Admin time Only one 1/2 day on Saturday once a month which you will have time off during the week Call by phone only 1:7 - one week after 5pm every 7 weeks, very light call Out patient only Reasonable patient volume $185k-$195k alary with bonus structure $12k quarterly production bonus $10,000 sign on bonus $5,000 retention bonus Paid malpractice insurance with tail coverage 3 weeks paid vacation to start 80 hours PTO CME - one week paid plus $2,000 Health, life, vision, dental and disability insurance 403b retirement plan Federal and State Student Loan Repayment Please call send us a copy of your CV Debbie Aber Physician Services 847-541-9347
