RN Care Manager

Sanford Health, a leader in healthcare services across multiple states with an expansive network including clinics, hospitals, and senior living facilities, is seeking a dedicated RN Care Manager to join our compassionate team. The RN Care Manager will be instrumental in enhancing patient care quality and effectiveness by coordinating comprehensive healthcare services. This position involves working closely with patients, families, and healthcare professionals to ensure patient-centered care across various settings within our system. Assess patient needs and develop personalized care plans. Coordinate with healthcare professionals to ensure seamless care delivery. Monitor patient progress and adjust care plans as needed. Educate patients and families on health conditions and care requirements. Manage a caseload of patients across Sanford Health facilities. Assist in the implementation of health interventions and follow-up. Strong clinical skills in a healthcare setting. Excellent communication and interpersonal skills. Ability to manage multiple cases effectively. Proficient in electronic health record (EHR) systems. Leadership and teamwork capabilities.
