Details - Home health setting serving Medford, OR and surrounding areas - 8-hour day shifts, 5 days per week (4x10 and 3x12 schedules are not available) - Patient visits up to 6 patients per day with a standard productivity expectation of 30 points - 1:1 patient care model - Charting via Epic and OASIS - No weekend or on-call requirements - Travelers are expected to float between all facilities within the state Requirements - Minimum 1 year of skilled home health PT experience required - BLS certification required - Epic EMR experience required - OASIS experience required - Reliable personal transportation required - COVID-19 primary vaccination series plus at least one booster required - License required if applicable - Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting Additional Information - First-time travelers will not be considered for this position - Local candidates within a 50–75 mile radius are accepted at the same rate - Former permanent staff must wait at least 3 months before returning as a traveler and must reside more than 50 miles from the submitting facility - Candidates with a pending license will be considered - Any requested time off should be disclosed upfront, as post-offer RTO requests may be denied - The scheduling system groups patient visits to minimize drive time, though occasional travel of 50 miles may be required - Any color scrubs are acceptable - Free parking available American Traveler Job ID P-680832.
