Provide direct nursing care to palliative and hospice patients in their homes or inpatient settings Assess patient condition by monitoring vital signs, performing physical examinations, and obtaining patient histories Create and oversee implementation of individualized care plans based on patient needs and physician directives Administer medications, manage pain relief, and monitor patients' response to treatments Collaborate with multidisciplinary healthcare team members to coordinate patient care Educate patients and their families on disease processes, treatment plans, and end-of-life care options Ensure compliance with healthcare laws, regulations, and policies in the conduct of clinical services Proven ability to handle emotionally charged situations and uphold the highest standards of compassionate patient care Excellent assessment, diagnostic, and clinical skills Strong organizational and communication skills Ability to collaborate effectively within a multidisciplinary team Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license in the state of employment CPR certification Certified Hospice and Palliative Nurse (CHPN) certification preferred but not required
