Monitor patient vital signs and rhythms using telemetry equipment, noting significant changes and implementing timely interventions. Work collaboratively with the healthcare team to develop and implement tailored care plans. Educate patients and their families about post-discharge care, especially regarding cardiac health, medications, and lifestyle adjustments. Ensure documentation is accurate and reflective of the care plan and patient responses. Administer medications as per physician orders and monitor for efficacy and side effects.
