Position Summary Plans, coordinates and provides total patient care specific to diagnosis and age of patients served on a continuing basis, to obtain optimum health for patient and family, in accordance with accepted hospital standards and policy and the South Dakota Nurse Practice Act. Consistently employs patient and family-centered care to promote safety, quality and satisfaction. Essential Job Functions Contributes to the ongoing assessment of assigned patients utilizing age specific growth and development guidelines. Recognizes changes in the patients health status on a regular basis as evidenced by chart review/observation. Assists with patient/family teaching. Demonstrates the ability to appropriately assess patients, apply clinical interventions, evaluate outcomes, and implement change to the plan of care. Demonstrates the ability to recognize an emergency and initiate appropriate response. Initiates, confirms, or assists in the authorization process for all appointments, treatments and diagnostic imaging in conjunction with patient navigators. Performs nursing procedures and treatments according to nursing and unit specific standards, as evidenced by documentation and observation. Prioritizes incoming phone calls and faxes from patients/families and other medical personal and walk-ins. Screens patients/families for multidisciplinary service needs. Education Requirements Requirements: Active LPN license required.
